The Standard Customer Process

1. Your Customer Box will be delivered to your clinic within 24 hours of enrollment.
Your Box Will Include
- Step-by-step instruction guide
- Pre-labeled specimen cups
- Biohazard bags
- Prepaid and pre-labeled UPS shipping bags
- All the paperwork you’ll need!
2. Fill out and fax over your New Client Account Form and your Provider Test Profile Form. These forms are in your box. You may also download them from our Client Resources Page.
3. You will receive a unique user ID and temporary password to access our secure online portal.
4. Each time you collect a sample, fill out a Test Requisition Form. Attach a quick printout of your patient’s demographics and insurance information from the system used in your clinic. Place your specimen and paperwork in the provided bag, and send it back to N3 Laboratories for testing.
5. You will receive your concise assessment report within 48 hours. You will receive your results through our secure online portal, or by fax, email, or mail. Choose the delivery option and report format that works for you!
